ATHLETICS CHECKLIST

Download Athletic Sports Paperwork or pick up paperwork from the Student Activities Office or at the front desk in the main building.

  1. Need Complete Physical Exam - Doctor’s signature on our form (One per school year – June 1st to June 1st)

  2. Sign Consent Form, Concussion and Physical Form - Parents and athlete

  3. Sign Steroid Code of Conduct Form - Parents and athlete

  4. Provide Medical Insurance Information

  5. Complete and Sign Transportation Declaration

  6. Turn Completed Forms into Student Activities Office

Athletes cannot participate until numbers 1-6 above are completed.

DO NOT WAIT UNTIL THE LAST MINUTE!  The Student Activities Student Payment Offices is open from 7 to 3:30 Monday through Friday (school days).

Must be Academically and Behaviorally Eligible and have no hours. 

Did your child TRANSFER to SBHS in the last twelve months? Complete CCS Transfer Paperwork with Athletic Director for Residential Eligibility.

Start Dates:

  • Fall Sports---August 10, 2012
  • Winter Sports---October 29, 2012
  • Spring Sports---January 28, 2013

There are many choices on the Transportation Page in the Athletic Paperwork... if you have questions, please call Mrs. Doty @ 637-5831 ext. 351 to clarify.

The Athletic Paperwork above contains the required documents for athletic participation at San Benito High School. Turn in the completed and signed forms to Mrs. Doty at the Student Activities and Athletics Office, room 351 before trying out for a sport. Electronically signed forms will not be accepted.
